The 5 year series of immunizations can be received anytime after 4 years of age. After school age, based on your child's needs, we recommend a physical exam every 1-3 years.
1. Dtap/HB/IPV is a combination vaccine called Pediarix.
2. MMR/Varivax is a combination vaccine called ProQuad.
3. If Hepatitis B series has not been received earlier, then it should be started at this time.
4. Given to a target population, per questionnaire.
5. The Prevnar and Rotavirus are recommended but not required vaccine for school attendance. Schedule may vary.
6. Hep A is a recommended but not required vaccine. Two dose series, administered at least 6 months apart.
7. Varivax booster given between 4-18 years of age. Recommended but not required for school attendance. May be given in conjunction with MMR as PROQUAD with 4-6 year vaccines or individually if MMR has already been administered.
8. One dose of Menactra (meningococcal vacine) is recommended between 11-18 years of age. A required vaccine for most colleges.
9. Booster dose to Dtap. Tdap is administered no sooner than 5 years after Dtap and not greater than 10 years.
10. HPV (Human Pappillomavirus) is a vaccine called Gardasil. 3 dose series for females only. Recommended but not required for school attendance.
11. Hep A series administered if not given earlier.

* "Catch up" vaccines if not given at 11-12 years of age.
